Sliding Fee Discount Program
We offer a Sliding Fee Scale to patients based on family size and household income, ensuring that no one is denied services due to an inability to pay. A minimum co-pay of will be collected during your visit.
If you arrive at your appointment and do not have proof of income, you will be asked to complete a 10-day slide fee form. You will need to provide proof of income within that time frame or you may be responsible for additional standard charges.
Zero Income Policy
If you do not currently have any source of income, you may complete our Zero Income Statement Form. This statement is active for 30 days, during which you will be required to apply for medical insurance or state medical assistance (Medicaid).
340B Pharmacy Discount Program
Through the federal 340B Drug Pricing Program, eligible HAHC patients have access to significantly discounted outpatient medications. Ask your care provider during your next visit if your prescriptions qualify for our pharmacy network savings.
Medicaid Application Submissions
Our patient navigators are certified and available to assist you with completing and submitting your Pennsylvania or Maryland Medical Assistance (Medicaid) application. Call our main office to schedule a free navigator session.
