Appointments & Cancellations

Please call 814-263-5804 to schedule appointments. This helps us manage clinics efficiently. If you are a new patient, please arrive 20 minutes prior to your scheduled time to complete necessary medical, dental, and demographic forms.

Cancellations & No-Shows: To respect our staff and doctors, a 24-hour notice is required if you are unable to keep your appointment. Any appointment not cancelled with proper notification will be considered a "no-show".

Prescription Refill Policy

Prescription refills are issued during office hours only. Contact your pharmacy and ask them to fax the refill request to our office at 814-263-5804. If your prescription has expired or you have not been evaluated recently, we will need to schedule an appointment before renewing the medication. We do NOT replace lost or stolen prescriptions.

Confidentiality & CRISP

Information is not provided to third parties without your explicit authorization. HAHC participates in the Chesapeake Regional Information System for our Patients (CRISP), a regional health information exchange. Your health information is shared securely with CRISP to improve care coordination.

You may opt-out of CRISP data sharing by calling 1-877-952-7477 or visiting www.crisphealth.org.

As an HAHC patient, you have the right to:

  • Receive quality care regardless of age, sex, religion, national origin, sexual preference, disability, or ability to pay.
  • Information contained in your record and active participation in care decisions.
  • Personal privacy and confidentiality regarding examinations and consultations.
  • Be evaluated and treated in a safe, clean, and professional environment.
  • Have advance directives honored and designate health proxies.
