VA CCN Program Overview

Under the Community Care Network, veterans who face long drive times or wait lists at standard VA facilities can obtain authorization to receive primary care, dental care, and behavioral health services from certified local community providers like HAHC.

How Do I Schedule an Appointment?

To establish care with HAHC through the Veterans Community Care Program:

1. Contact your VA primary care team or local VA facility case manager.
2. Request a referral to Hyndman Area Health Center for your desired service.
3. Once the VA issues the community care referral and authorization, our scheduling staff will contact you to set up your initial visit.
