Occupational Health Registration (Revised 20250327)
New occupational health patients and employer-referred employees can complete the official Occupational Health Registration Form prior to their visit. Includes employee demographics, employer authorization, pharmacy choice, assignment of benefits, and the HAHC Patient Safety & Code of Conduct Agreement.
